Case Management
Case Managers coordinate a multi-disciplinary team to provide continuity of care based on recovery principles. This is accomplished in a planned, efficient and effective manner, working in partnership with the individual.
Case Management services are provided to individuals in supported housing situations as well as to those individuals who are residing independently in the community, suffering with a serious mental illness that are receiving a service in the community by the agency.
